WHAT WE SUPPORT
Roots & Roads Community Hospice Foundation programs support individuals through the end-of-life process and provide education to the community about end-of-life issues.

Patient Supplemental Support
Roots & Roads is committed to providing support to hospice patients of limited means and their families. Support includes rent and utility assistance, personal items, food, and transportation.

Community Support
Roots & Roads provides bereavement books and journals to those grieving the loss of a loved one and participating in Casa de la Luz Hospice support groups, meetings, and classes, which are free and open to the public.
Roots & Roads also supports the We Honor Veteran’s program, which celebrates service members by providing special blankets from their branch of the armed forces as well as a framed certificate presented during a special ceremony.

End-of-Life Research
Roots & Roads supports projects which propose new approaches to care that improve the end-of-life journey.

Community Education
The Foundation seeks to educate the community through its annual End-of-Life Conference as well as other educational activities, promoting a quality end of life experience for all.

Advance Directives
End-of-life decisions do not have to be confusing. Roots & Roads Community Hospice Foundation provides a document called Five Wishes, free of charge, to help individuals explain their healthcare choices. This advance directive document is recognized in multiple states and clarifies an individual’s medical, spiritual, emotional, and personal needs and wishes.
