Your donation to Roots & Roads qualifies as an AZ Charitable tax credit. Contributions made between 10/13/2023 and 4/15/2024 can be claimed on your 2023 taxes. The tax credit is claimed on Form 321. Roots & Roads' QCO code is 22655.
This donation is a dollar-for-dollar tax credit on your Arizona state taxes.

2024 End-of-Life Virtual Conference Series
Through your partnership, Roots & Roads provides financial assistance during end-of-life care to individuals in need and their loved ones. Assistance includes personal items, rent and utility payments, final arrangements, and other support that provides comfort and dignity to patients.
Roots & Roads promotes quality end-of-life care by funding projects such as music therapy and non-pharmacological interventions for agitation. Studies reveal a profound impact on quality of life by supporting new approaches to care.
