2024 CORE Grant Awardee!

We are thrilled to announce that we have been awarded the 2024 CORE Grant from the Community Foundation for Southern Arizona! This marks the second time we have received this grant, and we are grateful for CFSA's ongoing support, trust, and collaboration as we work to transform the end-of-life experience for all members of our Southern Arizona community.


Roots & Roads is here to walk with you through the journey, no matter where you start.

We all deserve to have easily accessible, accurate, and unbiased information regarding serious illness and end-of-life care. In bringing you this information, we hope to ease some of the burden of what can be a difficult journey. 

Carrying Our Grief
2024 End-of-Life Virtual Conference Series


Through your partnership, Roots & Roads provides financial assistance during end-of-life care to individuals in need and their loved ones. Assistance includes personal items, rent and utility payments, final arrangements, and other support that provides comfort and dignity to patients.


Roots & Roads promotes quality end-of-life care by funding projects such as music therapy and non-pharmacological interventions for agitation. Studies reveal a profound impact on quality of life by supporting new approaches to care.
